EASTLAKE OAKS HOMEOWNERS ASSOCIATION, INC.

REQEST FOR HOME IMPROVEMENT

In an effort to protect each individual homeowner's rights, as well as those of the Homeowners collectively, it is a requirement oof the Association that any Homeowner, or group of Homeowners, considering improvements to, or alternations to the apperance of their real property submit a " REQUEST FOR IMPROVEMENT APPROVAL" to the Architectural Control Committee. This requires MUST be in writting and MUST BE APPROVED IN WRITTING PRIOR to the commencement of work. Improvements requiring prior approval include, but are not limited to: Patio covers kanais, out buildings, fences, landscaping, mailboxes, exterior paint, pools decks outside walkways, driveways and any other exterior modifications.

The Associaiton reserves the right to ask any homeowner to remove any architectural change made WITHOUT PRIOR APPROVAL AND/OR I N VIOLATION OF THE DECLARATION OF COVENANTS AND CONDTIONS AND RESTRICTIONS. The Association also reserves all rights under the law to enforce said Covenants, Conditions and Restrictions. Please submit completed request along with a sketch of the proposed omprovements on a site survey or plot plan and a $5.00 check to "Eastlake Oaks HOA" for processing your request:

I agree NOT TO BEGIN WORK on improvements UNTIL I AM NOTIFIED IN WRITTING OF THE APPROVAL OF THE BOARD OF DIRECTORS AND /OR ARCHITECTURAL CONTORL COMMITTEE I also agree to provide the County's final Inspection Report within thirty (30) days after the completion of the reqested work.
